Referrals and Initial Contact

We prefer our patients to have a referral from their general dentist so that we have current dental details. It may also mean that your Health Insurance rebate is slightly higher.

When you contact us to make an appointment our front office team will forward an information package to you. This package contains a Medical and Dental History form, information associated with the Privacy Act and a map showing the location of our practice in Joondalup. We would appreciate you bringing the completed Medical and Dental History form to your initial appointment.
